ASC Daily Briefing — September 10, 2026

New Today

QUAD A published a survey-findings advisory on radiology safety (September 9) — recurring deficiencies in personnel qualifications, PPE, equipment controls, and dosimetry. QUAD A reports that recent surveys keep turning up the same gaps: a facility passes equipment inspection and issues badges, but daily practice falls short. Surveyor focus areas:

  • Qualified personnel — only individuals qualified under state law operate, move, reposition, or adjust C-arms, X-ray tables, and portable units. Keep a current copy of applicable state radiology law readily available to the surveyor, verify and file credentials, and document training and competency validation where state law allows non-credentialed staff to perform limited tasks.
  • Staff safety and PPE — selection, use, storage, and condition reinforced through onboarding and periodic training, with availability checks built into daily workflow.
  • Equipment readiness — shielding and safety features verified against state/local requirements and manufacturer IFUs as part of room-readiness checks; radiation signage posted where visible to anyone approaching the area.
  • Dosimetry — policies covering badge assignment, monitoring, and storage, and exposure record retention of at least three years when X-ray is used. Where state and QUAD A requirements differ, follow the stricter one.

If you run a C-arm (pain, ortho, spine, GI fluoro), this is a cheap pre-survey audit: pull your state radiology regulations binder, the credential file for every fluoro operator, the PPE inspection log, and the last 36 months of dosimetry reports. This applies directly to QUAD A-accredited centers; the same elements sit under 42 CFR 416.49 (radiologic services) for AAAHC and Joint Commission deemed-status surveys.
