Accreditation Commission for Health Care (ACHC, formerly HFAP) was omitted. ASCA’s current roster confirms five national ASC accreditors: AAAHC, ACHC, DNV, The Joint Commission, and QUAD A. Correction — the ASC prior-authorization demonstration is 10 states, not Florida-only, and its start dates were delayed and phased. CMS delayed the demonstration from its original December 15, 2025 start and rolled it out in two phases across Arizona, California, Florida, Georgia, Maryland, New York, Ohio, Pennsylvania, Tennessee, and Texas. Phase 1 (CA, FL, GA, MD, NY, PA, TN) applies to dates of service on or after January 19, 2026; Phase 2 (AZ, OH, TX) to dates of service on or after February 16, 2026. 41 codes are subject to the demonstration — blepharoplasty/blepharoptosis/brow ptosis repair, botulinum toxin injection, panniculectomy and excess-skin excision, rhinoplasty, and vein ablation.
